Backline launched over a decade ago as a secure messaging platform for healthcare teams. We've spent those years inside hospitals, behavioral health centers, skilled nursing facilities, and physician groups — watching how care actually gets coordinated.
What we kept seeing wasn't a messaging problem. Messages were moving fine. The problem was everything that happened — or didn't — after the message was sent. Who was accountable? What was the deadline? What happened when no one responded?
That's the problem we're built to solve.

Most healthcare communication tools were designed to move information faster. Messages delivered. Alerts sent. The assumption: if it was received, it was handled.

Backline Pathways was designed to ensure work gets done. A message that confirms delivery isn't the same as a task that confirms completion. We built Pathways because "we know the message was sent" wasn't enough.
How we built it
Built with healthcare teams. Not for them — with them.
Pathways wasn't designed in a vacuum. Before we wrote a line of code, we spent months in working sessions with CNOs, CIOs, VPs of Operations, pharmacy directors, and case managers at academic medical centers, community hospitals, behavioral health organizations, and post-acute facilities.
We asked them where coordination actually breaks down. They told us about the discharge that took eight extra hours because no one could see what was blocking the bed. The transplant medication that sat unacknowledged in a thread. The consult that arrived 44 minutes late because paging confirms delivery, not response.
